Cost Estimates

If you have a dental benefit plan we are happy to assist you by filing your claim for services rendered. We will do our best to estimate your copay and out-of-pocket costs as closely as possible with the information provided by your benefits company.

Because dental benefits policies are a contract between the benefits company and yourself or your employer, we cannot guarantee these estimates. Although we are happy to assist you in understanding your benefits based on the information we have, we highly recommend that you contact your benefits company prior to receiving treatment.

We expect payment of your portion on the day treatment is rendered.

If you have insurance, please provide us with the following prior to your first appointment:

  • Name of Benefits Provider
  • Provider phone number
  • Employer providing the plan (if applicable)
  • Policyholder/Subscriber ID
  • Patient date of birth

To save time, you may fill out New Patient Forms and bring them with you. Please arrive 15 minutes prior to your appointment if you wish to fill out these forms at out at our office.
